If you are planning to use your insurance benefits, please book a Free Discovery Call (15 minutes) to discuss insurance coverage for your health condition. If you have any questions, feel free to email through the Contact Form or book a Free Discovery Call. Otherwise, you can schedule an Initial Consultation (60 or 90 minutes).
INSURANCE
At this time, I am in-network with Blue Cross Blue Shield, Cigna, True Health New Mexico, and United Healthcare, and always working to be adding to more networks. While I may be a network provider for your insurance company, your specific plan determines your coverage for nutrition services (coverage, deductible, co-pay, co-insurance, etc). Most plans cover only specific services and certain diagnoses as determined by your specific insurance plan. If you plan to utilize your health insurance for your visits, please schedule a Free Discovery Call to discuss further.
REFERRALS
If you plan to utilize your health insurance for your visits, I require a referral on file from your doctor prior to scheduling your appointment, for insurance documentation purposes. Please request your doctor fax a referral, including your latest medical visit notes with ICD-10 diagnosis code, and any pertinent labs, to: 505-221-5434.
If you do not plan to utilize your insurance, a referral with your latest medical visit notes and pertinent labs, are still helpful to me in understanding your health.
SELF-PAY OPTION
If your insurance plan does not cover nutrition services, I can provide you with a "Superbill" to submit to your insurance as an out-of-network provider. Nutrition counseling services are also considered reimbursable expenses under a Flexible Spending Account (FSA) or Health Savings Account (HSA).
